Ashifa Trivedi is the Lead Paediatric Pharmacist for the Paediatric Assessment at Home (PATCH) service across North West London. Having qualified as a pharmacist in 2004, Ashifa has worked across a range of paediatric specialties and currently provides strategic and specialist oversight to four PATCH teams based at Chelsea and Westminster Hospital, Imperial College Healthcare, London North West University Healthcare, and Hillingdon Hospital.

PATCH is a multidisciplinary hospital-at-home service that aims to reduce paediatric admissions by providing timely assessment and treatment in a child’s home environment. Since joining the service, Ashifa has led on harmonising clinical and operational guidelines across sites, strengthening medicines governance, and supporting the safe and effective use of medicines outside of hospital.

Ashifa is passionate about improving patient pathways and has a particular interest in supporting safe transitions of care, service development, and medicines safety.
